The thing is how is Sam going to realize, because Dean very rarely tells him. The only time he's heard it is through the demon and considering that they don't trust demons very much, I would guess that Sam might have taken it into consideration but at the same time not realized how much Dean needs him. I don't think Sam or Dean have particularly high self-worth (although of very different kinds) and I think this feeds into that.
