Those questions are kind of immaterial. I didn't want to waist my time on figuring that stuff out and weighing the narrative down with it. BUT, I can make something up on the spur of the moment.
Sam and Dean are visiting a witch's house, and the door frames are really low. Sam is not paying attention and walks into the door frame. The entire house falls in from the force of his head. The witch is not happy. Sam winds up female and thus shorter. Eventually, the spell expires when the witch realizes he's still taller than her door frames.
And what does Sam look like? Uh, think of young Jared, and then feminize it a bit. I think he'd be really quite impressive.
